In regards to the June 10 Standard PA Form for Medical Advisory Group please see attached for the final instruction sheet and prior authorization form for medical services.
- The advisory committee agreed that any penalty for not using the form shall be at the discretion of the Insurance Commissioner
- This will be the final meeting of the Advisory Group, pending any unforeseen problems with the form
