School districts must submit the following annual and biennial reporting forms and responses to questions using information from the previous tax year (calendar 2023) to the Comptroller’s office by Aug. 15, 2024, using these forms:
- Form 50-772-A (PDF) – Annual Eligibility Report
- Form 50-827A or Form 50-827B (XLSXs) – Biennial Cost Data Request (CDR)
- Biennial Progress Report Form questions (PDF) – New online reporting structure
School districts should have received all three forms from 313 agreement holders on June 17, 2024. School districts are required to ensure the completeness and accuracy of the information on the forms. School districts must submit PDF scans of the completed and signed Annual Eligibility Reports with attachments to the Comptroller’s office.
